You must have an order from the treating Physician and be there at 7am.

1. If it is for Primary care, you must present a medical report, medical order, copy of the ID cards or credentials and an order from the insurance company to process the approval and activation.
2. If it is for preoperative testing, the patient must contact the admission department and present a guarantee letter issued by the insurance company to proceed with the admission and further examinations.
